In the K menu, I first unlock the Widgets, then right click on
the item in the menu and it gives me the option to make
an icon on the desktop.

Unfortunately right click on Desktop, Create New -> Link to Application
doesn't exist.

I see a menu with the following choices only:
Run command
Add Widgets
Add Panel
Remove Desktop activity
Lock widgets
Lock Screen
Leave
Desktop activity settings.

Running Kubuntu 10.04.

Go to "Desktop activity settings" and change to activity type to
"Folder View" (or whatever the english version is). Then you'll have
a more classical desktop where you can create stuff like application
links.

Alternatively, without switching activity type, you can use the
Quicklaunch widget. Use the 'Add Widget' entry in the menu to add it.

This works fine in my KDE 4.5 PPA install.

You could try moving ~/.kde to some backup location (say ~/.kde.orig)
and try again with a clean slate. I, too, have 4.5 installed and not
nearly all the problems you describe.

You have to right -click and select 'unlock widgets' to be able to change from
'desktop' to 'folder view'. Also unlock widgets in order to add an icon.
