Default dm log userspace check additional error conditions

Patch name: dm-log-userspace-check-additional-error-conditions.patch

This patch resolves unhandled errors.

When constructing a mirror, it is possible for the initial request to
fail for other reasons besides just -ESRCH. These must be handled too.
