device-mapper errors during pvmove

I am trying to remove an old physical disc from my logical volume. I have had earlier problems, as discussed in recent days on the lvm mailing list, but it is now suggested that I post here. The LV has sufficient free PEs on it, but pvmove is not working because of device-mapper errors. I have added a temporary usb external hard drive as it appears that pvmove needs to create a mirror lv temporarily.
