Are you using a stock kernel, or a self-built one? 'uname -a'?

You need to install the firmware-ipw2x00 package from non-free.

I assume that that's a stock kernel. What does
'/usr/lib/modules/2.6-686/' contain? You seem to be missing the
module; I have no idea why.
