I don't have an /etc/modules.d/alsa file on squeeze.
I have the following alsa files in /etc/modprobe.d/
alsa-base-blacklist.conf
alsa-base.conf
There is an options snd-usb-audio index=-2 line in alsa-base.conf .
Changing -2 to 0 is not sufficient to get the headphones working.
In fact it doesn't seem to have any affect on the sound at all.
There is no snd_ac97_codec index line in the file.

Pulseaudio looks interesting, but it's still in the fermentation stage.
This is a production machine and I'm running squeeze rather than wheezy
or sid. I rely on it every day and can't afford to have things screwed
up. Perhaps it will be stable next year. But thanks for the suggestion.

I've been pretty happy with PA since starting to use it. Squeeze was
released in February and I have been using pulse for much longer than
that.

I just plugged in a "Hercules USB 5.1 Channel Audio Adapter" and it
works like a charm.
I tried with both pulseaudio
0.99.2-2
0.9.21-4

I only used the "front audio", so I can't comment on its 5.1-ness.
