During a FAI controlled installation of an HP ML350 G5 server, I create
disks on the raid controller using hpacucli, like:

When I do this with the 2.6.39 kernel from squeeze-backports, I get
the following stack-trace:

The problem seems to be that cciss_unlocked_open() is called through the
sequence do_ioctl() -> rebuild_lun_table(). So the driver is locking
against himself. The following patch fixes it:

Thanks for a clear report. There have been some cciss fixes upstream
recently, though none of them seem particularly relevant. Please test
v3.1-rc1 or later from experimental --- it should not require any
packages from outside squeeze except for linux-base and
initramfs-tools (which you can get from squeeze-backports).

If that fails, please send a summary of your findings to Stephen M.
Cameron <scameron@beardog.cce.hp.com>, Mike Miller
<mike.miller@hp.com>, and iss_storagedev@hp.com (addresses taken from
MAINTAINERS), and cc this bug log so we can track the resulting
discussion.

I will see if I can make a setup for this....

As an extra datapoint, I had also tried the cciss-4.6.28-14.tar.bz2
from http://sourceforge.net/projects/cciss/files/cciss-4.6-tarballs. I
had to patch that source for the removal of the 'big lock' and that
driver had the same problem. Which is logical (of course in retrospect
;-) because the problem seems to be caused from replacing the big-lock
on the ioctl-function by the mutex_lock/unlock on cciss_mutex...

I tried to reproduce the problem with 3.1.0-rc7-686-pae from
experimental as you requested, but I failed. The problem is that
hpacucli does not find any controllers with this kernel :-( I used the
latest available hpacucli version (8.75-12.0). This version definitely
works with 2.6.39. I tried strace-ing hpacucli but that didn't give me
any clues. It was looking for /dev/cciss/c0d0/cciss, which is weird
and might indicate that the 3.1 environment makes the hpacucli device
search fail....

Anyway, I have a 3.1 setup now, but unless I can get hold of a working
hpaccli, I will not be able to test this bug on 3.1.

Yeah I suspect I know what's going on. There was some code
in hpaaculi which was asking the question "Is this a 2.4 kernel?
Or is this a 2.6 kernel?" It didn't consider the possibility of
a 3.x kernel, and in the face of a 3.x kernel, it just kind of
gave up.

Pretty sure this bug is fixed in a released version of hpacucli
because rhel6.1 uses a 3.x kernel, and I know we support that.

Bill Kilcoyne might know the specifics.

Er, no rhel6.1 doesn't use a 3.x kernel. I think I must have
been thinking of rhel 6.2, which isn't out yet.

RHEL 6 will be sticking with what is nominally version 2.6.32, although
RH continues to backport large changes from later versions. The first
stable distribution with a 3.x kernel will presumably be Ubuntu 11.10
'oneiric'.

Stable update 3.0.4 added a process personality flag that causes the
kernel to report version numbers beginning '2.6.', and the 'setarch'
command in util-linux has been updated to allow setting this flag.
However. neither Debian nor Ubuntu has this new version of util-linux
yet.

It's sles11 sp2 that
uses a 3.0.x kernel.
