Default Kill off all the old kernel-xen hacks

Hey,
Now that kernel-xen is gone, I think it makes sense to remove
all the special hacks we had in place for it.

I've gone through each of the hacks one by one and the removal
patches which follow give the rationale for removing each individual
hack.

Note, the patches aren't tested because I've completely failed
in my attempts to locally build install images from rawhide.
