dpkg -l util-linux

ls /etc/initramfs-tools/conf.d/

change MODULES=DEP to MODULES=MOST

Could you please paste the line with the PATH settings
from /root/.bash_profile and the
#echo $PATH output.
(1)

Another useful output will be:
#vol_id --uuid /dev/sda5
#vol_id --uuid /dev/sda6
(2)

and like Maximilian said, the
#cat /proc/mount
(3)

/etc/initramfs-tools/conf.d/driver-policy
